Annual Registration Forms

Landmark School requires parents to complete a set of registration forms each year a child attends the school. These are separate from the initial application and contract (Enrollment Agreement). Landmark is now using an electronic enrollment system to streamline the process of completing and processing the forms. The medical forms still need to be completed on paper and mailed.

Electronic Registration Forms

Once admitted, one parent in the child's custodial family receives an email notification with a unique username and password.

Paper Medical Forms

These forms must be completed and returned to the Health Center at your campus before July 15, 2013 (or June 17, 2013 for the Summer Program.)

Please download, print, complete, and mail the following documents:

  1. Parental Authorization to Treat Form
  2. Physical Examination & Immunization Form Form must be signed by physician.
  3. Dental Health Form Form must be signed by dentist.
  4. Medication Administration Form
  5. Copy of both sides of Your Health Insurance Card & Prescription Drug Card
  6. If required: Asthma, Food Allergy, & Bee Sting Allergy action plans signed by healthcare provider.
